North Coast Health Ministry to Begin Offering Appointments at Westown Physician Center in Cleveland |

Beginning January 25, 2012, North Coast Health Ministry (NCHM), a free clinic serving the low-income uninsured, will offer primary care appointments at the Westown Physician Center at 10654 Lorain Avenue in Cleveland. Appointments must be scheduled in advance and will take place on Wednesday and Thursday mornings beginning at 8:30 a.m.
“Our provision of care at this site will allow us to bring care closer to some of our patients in the city of Cleveland,” says NCHM Executive Director Lee Elmore. “Many of our patients have transportation issues, and our hope is that, by offering our services in this in this area, more low-income, uninsured patients will be able to readily access the care they need, despite the transportation challenges they face.”
The space at Westown Physician Center is being donated for use by Fairview Hospital. North Coast Health Ministry will also continue to provide patient care at its primary location at 16110 Detroit Avenue in Lakewood five days per week.
“We are extremely grateful to Fairview Hospital for their donation of this space and their commitment to meeting the health needs of the underserved in the community,” says Elmore.
Individuals receiving care at either the Westown or Lakewood locations must be registered patients of North Coast Health Ministry. North Coast Health Ministry serves people earning up to 250 percent of the federal poverty level (income level varies based on family size) within 17 zip codes in northwestern Cuyahoga or northeastern Lorain counties. North Coast Health Ministry Offers Benefit Bank Counseling |

North Coast Health Ministry is now offering assistance to members of the community wishing to apply for public assistance programs through The Ohio Benefit Bank™. Individuals interested in learning about programs they may be eligible for are guided through an online application process by a trained Benefit Bank counselor. These programs can provide resources to individuals who may be having a hard time making ends meet, and include food and nutrition, health care, energy and child care assistance.
“It is our job as a free clinic to help our patients get access to all forms of assistance for which they are eligible,” says NCHM Executive Director Lee Elmore. “If someone is eligible for Medicaid, for instance, it’s our responsibility to assist them with their application so that they can get the benefits to which they are entitled. Our Ohio Benefit Bank counselors can help them through the process and, in many cases, help them access a range of other supports that positively impact their health, such as food, prescription, and utility assistance.”
Each year, more than $2.4 billion in public assistance goes unclaimed by Ohio citizens. Nearly 97 percent of these dollars are federal funds and must be returned for reallocation to other states or programs. The Ohio Benefit Bank™ strives to assist Ohioans in accessing these dollars because they are revenue for our local economies, they help stabilize families, and they help low- and moderate-income people overcome the devastating effects of poverty.
North Coast Health Ministry’s Benefit Bank counselors are available by appointment only and takes place at 16110 Detroit Avenue, Lakewood, Ohio. For many of the programs supported by The Ohio Benefit Bank™, certain income guidelines must be met. Other programs have income guidelines as well as resource guidelines that must be met. Individuals that believe they may be eligible for assistance and are interested in scheduling an appointment with a counselor should call 216-228-7878. |

Been selected as a 2010 Plain Dealer Holiday Spirit Agency. NCHM was one of local 20 agencies providing health and human services chosen from a total of 87 local agencies that applied for the program.

Received a grant from the office of Ohio Attorney General Richard Cordray for patient prescription medication assistance. The one-time grant is the outcome of a 2008 multi-state settlement with Express Scripts Inc.

NCHM Partners with Discount Drug Mart on Patient Prescription Medication Program |
News Release
CLEVELAND, Ohio - March 4, 2009 - North Coast Health Ministry (NCHM), a free clinic for the low-income uninsured on Cleveland's West Side, has partnered with Discount Drug Mart as the exclusive provider of prescription medications for NCHM patients. Through the arrangement, patients can purchase medications prescribed by a North Coast Health Ministry physician through a co-payment at any Discount Drug Mart store.
While NCHM works with its patients to enroll them in prescription assistance programs offered by pharmaceutical manufacturers through which they can receive free or low-cost medications, there are many drugs not included in the manufacturers' programs. Often, even for drugs included in the programs, there is a waiting period of a month before a patient can begin receiving free or low-cost medications. The partnership with Discount Drug Mart will allow patients with a NCHM identification card to access their prescriptions through a $25 co-pay for generic medications or a $50 co-pay for brand-name medications.

NCHM Selected as a Partner Agency for Hyland Software's Employee Volunteer Program |

CLEVELAND, Ohio - February 10, 2009 - Hyland Software, Inc. has selected North Coast Health Ministry (NCHM) as a partner agency for its employee volunteer program. The purpose of Hyland's newly formalized employee volunteer program is to give back to the communities where company employees live and work by offering paid time off for volunteer work.
North Coast Health Ministry was chosen for the partnership as a local non-profit organization that uses Hyland Software's OnBase enterprise content management (ECM) software and as an expression of Hyland's support for NCHM's mission of providing health care to the low-income uninsured.
