How To Become a Patient at North Coast Health Ministry

WHO DO WE SERVE?

Our patients are people who:
• Have no health insurance
• Are living on a limited income
• Live in northwestern Cuyahoga and northeastern Lorain counties

WHAT SERVICES DO WE OFFER?

• Primary health care (newborn - 65 years of age)
• Regular check-ups/preventive care
• Chronic disease management
• Medical tests
• Women’s health care, including mammograms
• Prescription assistance
• Referrals for specialty medical treatment
• Behavioral/mental health counseling
• Links to other services
• Healthy Start information and applications

 

Become a patient in three simple steps:

 

  1. Call NCHM at 216-228-7878 and asked to be screened as a new patient.  You will be asked questions about your household (family size and location) and income.  If, from this information, you appear to be eligible for North Coast Health Ministry services, you can either pick-up or be mailed a packet to complete.

  2. Return the information packet, along with proof of income and residency, in person or through the mail to become registered.

  3. After registration, you can call to schedule an appointment (at our Lakewood or Westown locations) or attend a walk-in clinic offered Monday through Friday at 8:30 AM (Lakewood location only).

 

Even if you do not have an immediate medical need, becoming a registered patient of North Coast Health Ministry will provide you with a medical resource should the need arise.

 

The Ohio Benefit BankTM

North Coast Health Ministry of Lakewood, a free clinic serving the low-income uninsured of greater Cleveland, is offering assistance to community members wanting to apply for public assistance programs through The Ohio Benefit Bank.

The Ohio Benefit Bank, implemented through a public-private partnership between the Ohio Association of Second Harvest Foodbanks, the State of Ohio and more than 1,100 faith-based and community organizations throughout Ohio, is an Internet-based program that assists residents in accessing federal public assistance funds through multiple agencies and organizations.

“It is our job as a free clinic to help our patients get access to all forms of assistance for which they are eligible,” said NCHM Executive Director Lee Elmore. “If someone is eligible for Medicaid, for instance, it’s our responsibility to assist them with their application so that they can get the benefits to which they are entitled.  Our Ohio Benefit Bank counselors can help them through the process, and, in many cases, help them access a range of other supports that positively impact their health, such as food, prescription and utility assistance.”
